| a. |
These forms are included in the application packet or may be filled in with an online Personal Reference Form. |
| b. |
One
Personal Reference Form must be completed by the pastor of the church
you are currently attending. The other two Reference Forms may be
completed by non-family members of your choice. |
Step 4Arrange to have an official
high school transcript sent to the Admissions Office. A Request for
High School Transcript Form is included in the application packet or
can be downloaded (PDF file) (73.71 KB) from this website.
Step 5Take the American
College Test (ACT) and have the score reports sent to the Admissions
Office. Because Northland participates in the American College Society
Program, high school graduates are required to take the ACT. The
Application for Admission may be submitted before taking the test. It
is recommended that the test be taken by November of the high school
senior year. A bulletin of information and a blank application with the
dates of administration and fees may be obtained by writing American
College Testing Program, P.O. Box 414, Iowa City, Iowa 52243. The ACT
code number for our institution is 4623. These test scores are useful
in processing a student's application and in advising him during his
college career. (NBBC may also accept score reports for the SAT in lieu
of the ACT.)
Step 6Complete a Medical History Form, Medical History Update, and a Consent to Examination Form; and submit them to the Admissions Office.

These forms may be attained by contacting the Admissions Department at 1-888-4-NORTHLAND or (715) 324-6900, ext. 3100. |
| b. |
Each applicant must have had a physical exam within 12 months prior to applying and must have all immunizations current. |
Step 7Arrange to have an official transcript sent to
the Admissions Office from each institution where you received
college-level credit. A Request for High School Transcript Form (73.71 KB)
Ordinarily, an application can be processed
in no less than three weeks; and it is not unusual for an application
to require six weeks or longer for processing. It is necessary to
secure the high school record, to evaluate any transcripts for credit
earned from other institutions, to secure character references, and to
coordinate all of these records in the Admissions Office before a final
letter of acceptance can be sent. The application deadline for the fall
semester is August 15; the deadline for the spring semester is December
15.
